Published in the Formosa Journal of Science and Technology, the research comes at a time when healthcare systems worldwide are facing increasing pressure from technological change, rising patient expectations, and the growing complexity of medical services. The findings are considered important because patient safety remains one of the most critical challenges in global healthcare.
According to the World Health Organization, unsafe medical care contributes to millions of preventable injuries and deaths every year. In many cases, patient harm is linked not only to clinical mistakes, but also to weak institutional governance, poor communication, and unclear legal accountability within healthcare systems.
The study conducted by Tiwuk Herawati from the University of Muhammadiyah Malang examined how healthcare governance and legal accountability interact to support patient safety in modern medical systems. The research found that hospitals with stronger governance structures and clearer legal procedures are more likely to create safer environments for patients.
The researcher explained that patient safety should not be viewed solely as the responsibility of doctors or nurses. Instead, it depends on how healthcare institutions are managed, supervised, and regulated at every organizational level.
“Effective governance and legal accountability create a culture of safety and transparency in healthcare institutions,” the study noted, emphasizing that organizational systems play a major role in preventing avoidable medical incidents.
Healthcare Governance Becomes a Key Factor
The research identified healthcare governance as one of the strongest factors affecting patient safety outcomes. Governance in this context refers to hospital leadership, decision-making systems, transparency, supervision, ethical standards, and institutional accountability.
The study showed that hospitals with better governance mechanisms experienced significantly higher levels of patient safety. Strong leadership and clear operational procedures were associated with better communication among healthcare workers and lower risks of medical negligence.
Legal accountability also played an important role. Hospitals that implemented stronger legal compliance, reporting systems, and professional responsibility standards were found to be more effective in preventing patient harm.
The research further revealed that legal accountability partially mediates the relationship between healthcare governance and patient safety. This means governance systems become more effective when supported by clear legal frameworks and consistent enforcement of healthcare regulations.
Research Involved Healthcare Professionals
The study used a quantitative explanatory research design involving 270 respondents from different healthcare professions. Participants included doctors, nurses, hospital managers, and administrative staff working in healthcare institutions.
Researchers used structured questionnaires with scaled responses to collect data. The analysis was conducted using a statistical modeling approach designed to measure the relationships between governance systems, legal accountability, and patient safety outcomes.
Most respondents were nurses, representing approximately 45 percent of the sample. Doctors accounted for around 30 percent, while hospital managers and administrative personnel made up the remaining participants. More than half of the respondents had over five years of professional experience, providing insight from healthcare workers directly involved in hospital operations.
The findings showed:

- Legal accountability also significantly improved patient safety outcomes.
- Governance systems became more effective when supported by legal compliance mechanisms.
- The combined research model explained approximately 68 percent of patient safety variations observed in hospitals.
These results indicate that patient safety depends heavily on organizational management quality, not only on individual clinical performance.
Growing Relevance in the Digital Healthcare Era
The findings are increasingly relevant as hospitals adopt digital technologies and artificial intelligence tools in healthcare services. While AI-based systems can improve efficiency and diagnostic support, they may also introduce new risks if hospitals lack clear governance and legal safeguards.
The research argues that modern healthcare institutions must strengthen internal oversight systems to ensure that emerging technologies do not compromise patient safety standards.
Hospitals are also encouraged to improve transparency in reporting medical incidents, conduct regular staff training, and maintain continuous evaluation of healthcare procedures. According to the study, these measures can help create a stronger culture of safety within medical institutions.
The study additionally suggests that policymakers should develop healthcare regulations that support accountability while encouraging innovation in healthcare technology.

The research from the University of Muhammadiyah Malang may provide practical guidance for hospital administrators, government regulators, and healthcare policymakers seeking to improve healthcare quality and patient protection systems.
For healthcare institutions, the findings underline the importance of integrating governance reforms with legal accountability measures. Hospitals that fail to build transparent and accountable systems may face greater risks of medical disputes, patient dissatisfaction, and reputational damage.
For the public, the study reinforces the idea that patient safety is not determined only by medical expertise. Institutional systems, leadership quality, and legal protections are equally important in ensuring safe healthcare delivery.
